We collect personal information directly from you wherever possible — for example, during enquiries, assessments, consultations and the delivery of care. With your consent, we may also collect information from your family or representatives, treating health professionals, hospitals, and funding or referral bodies.

We use your personal information to assess your needs, plan and deliver safe, high-quality care, coordinate services and funding, communicate with you and your representatives, and meet our clinical, legal and regulatory obligations.

We will not use your information for any purpose that is unrelated to your care without your consent, unless required or authorised by law.

To coordinate your care, we may share relevant information with carers and staff involved in your support, your treating health professionals, allied health providers, hospitals, and — where applicable — Support at Home, government departments and funding or care-coordination partners.

We only disclose the information necessary for these purposes, and we require those we work with to protect your information. We do not sell your personal information.

We take reasonable steps to protect your personal information from misuse, loss, unauthorised access, modification or disclosure. This includes secure physical, electronic and administrative safeguards, and limiting access to those who need it to provide your care.

We retain your information only for as long as it is needed to deliver care and to meet our legal and record-keeping obligations, after which it is securely destroyed or de-identified.

You have the right to request access to the personal information we hold about you, and to ask us to correct it if it is inaccurate, out of date or incomplete. To make a request, please contact us using the details below. We will respond within a reasonable time and in accordance with the APPs.

When you visit our website, we may collect limited technical information (such as your browser type and pages viewed) to help us improve our services. Our website may contain links to external sites; we are not responsible for the privacy practices of those sites.
